The Jacqueline Scholl Access to Care Fund is dedicated to ensuring access to high-quality auditory processing therapy for individuals who would otherwise be unable to afford services. This fund honors Jacqueline Scholl’s invaluable contributions to our clinic and her dedication to supporting individuals with auditory processing challenges.

Eligibility Criteria
To qualify for assistance through the Scholl Therapy Assistance Fund, applicants must meet the following criteria:
1. Insurance Limitations:
a. The applicant’s insurance must be out-of-network with the clinic, making therapy services ineligible for coverage.
2. Financial Need:
a. The applicant must have completed a financial hardship form and still be unable to afford the cost of therapy.
b. Proof of financial hardship may be required, including income verification, proof of government assistance, or a personal statement describing financial limitations.
3. Clinical Necessity:
a. The applicant must demonstrate a clinical need for auditory processing intervention, as determined by an initial evaluation or referral from a qualified professional.
4. Commitment to Therapy:
a. The applicant (or their guardian) must demonstrate a commitment to attending scheduled sessions, actively participating in treatment, and completing home practice recommendations.
b. Missed sessions without prior notice may result in loss of eligibility for continued assistance.

Program Structure and Session Allocation
Number of Donated Sessions:
Each recipient may be awarded a specific number of therapy sessions based on available funding. This will be determined by clinic leadership on a case-by-case basis.
Reassessment of Need:
Recipients may apply for renewed assistance after completing their initial allocation of sessions. However, continued support is contingent on funding availability and demonstration of progress in therapy.
Confidentiality:
All financial and clinical information will be kept strictly confidential in accordance with HIPAA guidelines.

Application Process
1. Submission of Application:
a. Applicants must complete and submit the Scholl Therapy Assistance Fund Application (available at the clinic or online).
b. The application must include supporting documentation demonstrating financial need and clinical necessity.
2. Review & Selection:
a. Applications will be reviewed by a designated fund committee on a rolling basis.
b. Decisions will be based on need, available funding, and clinical priority.
3. Notification:
a. Applicants will receive a written decision within 4 weeks of submission.
b. If approved, recipients will be informed of their session allocation and expectations for participation.
